To order a plaque to honour a loved one on the Light Up Your Life Memorial Wall, please follow these steps:

Paste the copied image into a Word document.
Print out the Word document and fill it out with the required information.
Attach a cheque or money order in the amount of $150 made out to Light Up Your Life Society. Please do not pay by cash.
Mail to the address on the order form.
(Alternatively, you may hand deliver the order form plus cheque or money order placed in a sealed envelope which bears the mailing address to the WestView Health Centre at 4405 South Park Drive, Stony Plain.)
